The 24/7 Nurse On Call line focuses on preventative health. Our dedicated health care professional will perform a detailed assessment of your current health to identify any risk factors and will convey issues that you may be having directly to your physician. Our nurse can asses your current situation and advise you on your next steps.
At Trinity Health Associates we provide care management services in which you have access to a dedicated health care professional 24 hours a day and 7 days a week. With this access our team is able to provide you with self-management education and support for any health issues. We keep you on track between doctor visits via the phone in addition to assisting with coordination and communication with your pharmacy, specialists, testing centers, hospitals and more.
